Requirements

Must have a current NYS LPN/RN License
Experience working with individuals with mental health conditions

Responsibilities

Duties include conducting health assessments, and other pertinent assessments or evaluations, as needed.
Administer injections and maintain medication logs.
Provide CT, CRS and IR services on an individual basis and class setting as needed (includes symptom monitoring, clinical counseling, relapse prevention, medication education, wellness self-management, basic living skills, clinical counseling, crisis intervention and other services.) Provide individual and group rehabilitation/recovery and guidance/empowerment in PROS program setting.
Complete documentation and follow up in accordance with regulations and daily operations.
Act as liaison with medical teams, other providers, insurance companies, pharmacies and labs.
Manage a caseload of clients, if needed.
Assist/empower individuals to identify and achieve recovery goals of employment, wellness, education and community integration.
Function as a vital part of an interdisciplinary team environment and provide team support. Participate in interdisciplinary team meetings and make recommendations as needed.
Receive supervision by and report to the Program NP and/or Program Director.
