The Telephonic Nurse Case Manager II is responsible for care management within the scope of licensure for members with complex and chronic care needs by assessing, developing, implementing, coordinating, monitoring, and evaluating care plans designed to optimize member health care across the care continuum. Performs duties telephonically. This position will service members in different states; therefore, Multi-State Licensure will be required. The position is virtual, and candidates must reside within 50 miles of an Elevance Health Pulse Point location. The working hours are Monday - Friday from 10:00am to 6:30pm EST, with 1 evening shift twice per month from 11:30am to 8:00pm EST.
