About The Position

This position involves triaging or managing symptom-based encounters with patients over the phone or via video, utilizing evidence-based triage protocols. The role aims to decrease unnecessary visits to healthcare providers and emergency rooms while providing patients with self-care information. The Triage Nurse will assess a wide range of patient conditions, from healthy individuals to those with acute or chronic illnesses, and will employ a structured nursing process to ensure effective patient care and advocacy.

Requirements

• Must be a graduate of a School of Nursing, BSN preferred.
• Must have a current, active RN license and hold or apply for a Nurse Licensure Compact license.
• Minimum of five years' experience as a registered nurse.
• Previous emergency department or telephone triage experience is desired.
• Experience in using computers and electronic medical records.

Responsibilities

• Accepts telephone calls from patients with varying levels of acuity and types of medical concerns.
• Promotes clear and coherent verbal communication following an appropriate line of questioning.
• Utilizes the electronic medical record to access and review patient records and triage protocols.
• Provides clear health information and assistance with medication refills as needed.
• Evaluates the significance of the patient's concern and determines the appropriate course of action.
• Completes Transitional Care Management telephone encounters for post-hospital patients.
• Functions independently in a fast-paced environment.
