The telehealth Patient care coordinator is to help medical staff with providing secretarial tele-health assistance during the delivery of patient are via telecommunication. Acting as a liaison between patients and healthcare providers to ensure smooth communication and coordination of care. Providing administrative support with access to forms, PHI, IT services, access to pts via tele-communication, filling and ensuring the provider has all needed documents and access to the treatment team as needed. Requirements

  • High School Degree/GED required; or Associates Degree in a Human Services field; or Bachelors Degree in a Human Services field; or a Certified Nursing Assistant.
  • One year of work experience in a behavioral health facility, or bachelor' s degree in social services or related field, plus one year of work experience in behavioral health service or delivery preferred.
  • WORK HOURS: 8 hour shifts. PRN employees must work four shifts per month, one shift must include a weekend.
