About The Position

The Team Registered Nurse at Hosparus Health is responsible for providing individualized hospice nursing care to patients facing advanced illness or end-of-life care. This role involves assessing patient and family needs, planning and implementing care, and coordinating with an interdisciplinary team to ensure high-quality service. The position emphasizes a holistic approach to care, supporting patients in various settings, including homes and healthcare facilities.

Requirements

• Current RN licensure in Kentucky and/or Indiana or ability to obtain upon offer.
• One year of nursing experience required.
• Knowledge of palliative care helpful.
• Valid driver's license in accordance with policy.
• Use of technology-based tools, including electronic medical records, strongly preferred.

Nice To Haves

• BSN desired.
• Certification in hospice/palliative care preferred.

Responsibilities

• Assess and identify patient/family needs.
• Plan, implement, evaluate, and document individualized hospice nursing care.
• Coordinate care among the Interdisciplinary Group and other medical providers.
• Supervise hospice care provided by Licensed Practical Nurses and Certified Nursing Assistants.
• Demonstrate excellence in nursing practice for assigned case load.
