About The Position

The Patient Care Services Director is responsible for overseeing all patient care services for the team, including volunteer and bereavement services, ensuring that these services are integrated components of the interdisciplinary plan of care. The director reviews information on all pending patient admissions, new admissions, and 'alive' discharges to ensure eligibility and appropriateness criteria are met. Collaboration with both the team physician and the attending physician is essential for palliative care measures related to pain and symptom management. The director ensures complete and accurate data and documentation are available for recertification decisions, which must be made in a timely manner. Additionally, the director monitors the type and level of team services provided to ensure they are appropriate and responsive to patient and family needs. The role includes managing the team schedule and coordinating services 24/7, facilitating team meetings, and participating in an on-call rotation.

Requirements

  • Reliable transportation with appropriate license and insurance coverage.
  • Private telephone in home.
  • Knowledge of primary medical and nursing care principles.
  • Working knowledge of oncologic nursing and palliative care.
  • Understanding of interpersonal dynamics within an interdisciplinary health care team.
  • Knowledge of the process of dying and bereavement.
  • Working knowledge of VITAS information system (Vx).
  • Two years successful supervisory experience in a health care organization.

  • Bachelor's degree preferred.
  • Current and valid R.N. License to practice in the state where the VITAS program is located.

Responsibilities

  • Direct all patient care services for the team, including volunteer and bereavement services.
  • Review information on pending patient admissions and new admissions to ensure eligibility and appropriateness.
  • Collaborate with team and attending physicians regarding palliative care measures.
  • Ensure accurate data and documentation for recertification decisions.
  • Monitor team services provided to assure appropriateness and responsiveness to patient needs.
  • Manage team schedule and coordinate services 24/7.
  • Facilitate team meetings for interdisciplinary input on care plans.
  • Participate in on-call rotation.
  • Interview, select, train, supervise, evaluate, and dismiss team staff.
  • Ensure staff competence through field visits and evaluations.
  • Oversee staff and volunteer schedules and territory assignments.
  • Review and approve payroll for team members.
  • Monitor clinical records and team activities for compliance with standards.
  • Develop and implement performance improvement activities.
  • Ensure completion of patient/family satisfaction surveys.
  • Address customer service issues and grievances promptly.
  • Analyze customer service issues to improve team performance.
  • Participate in professional, voluntary, or community service organizations.
  • Verify and approve admitting IPOC diagnosis, treatments, staffing, and supplies.
  • Monitor resource utilization for cost-effective service delivery.
  • Attend inservices, educational seminars, and workshops for professional development.
