About The Position

The OnTrackNY Program is an innovative treatment program at ICL. The Team Leader will work as part of a small multidisciplinary team providing coordinated and assertive treatment in the earliest stages of illness.

  • LMHC; LMSW; NYS licensed Clinical Social Worker or Ph.D clinician in appropriate field (Social work, psychology)
  • Two years of supervisory experience
  • Experience working with adolescents and/or young adults with serious mental illness.
  • Experience in community-based mental health services

Responsibilities

  • Provides overall supervision to the team and is responsible for allocation of staff time.
  • Ensures all team members are informed of issues involving participants and their families.
  • Monitors caseload size and coordinates program intakes and discharges.
  • Facilities weekly team meetings and provide training and supervision of the IPS specialist and recovery coach.
  • Makes rotating call schedule for after-hours coverage by team members.
  • Rotating on-call responsibilities with other team members.
  • Serves as the primary clinician for a subset of program participants.
  • Serves as the point person for the participant and their family
  • Conduct needs assessments
  • Engages in safety planning and wellness management planning with participants
  • Connects participants and families with the services and supports they need
  • Provide behavioral interventions and take an especially active role in the family support domain.
  • Conducts clinical assessment to determine the participant's level of social functioning and substance use via a shared decision making (SDM) framework
  • Provides Social Skills Training, Substance Abuse Treatment, Behavioral Activation, coping Skills Training, And Psycho-education.
  • Provides psycho-education regarding first episode psychosis (FEP) and the goals of the OnTrackNY program
  • Provides supportive therapy
  • Develop and modifies treatment using a shared decision-making process with clients.
