About The Position

The Team Leader (Club Erie) is a non‑exempt position reporting to the Program Operations Supervisor. This role is responsible for coordinating and supervising staff who provide community habilitation services to individuals supported by the Barber National Institute. Strong organizational, analytical, and interpersonal skills are essential, as well as the ability to support a collaborative, community‑integrated program. Team Leaders ensure compliance with all agency policies, regulatory standards, and the Barber National Institute mission, vision, values, and code of conduct. Requirements

  • High School Diploma or equivalent and 1–2 years of experience supporting individuals with disabilities
  • Must be at least 18 years of age
  • Understanding of Individual Support Plans (ISPs) and community habilitation services
  • Ability to complete timely and accurate documentation, including progress notes, billing, and reports
  • Familiarity with or willingness to learn CareLogic and Electronic Visit Verification (EVV) systems
  • Skills in scheduling, staff training, performance evaluation, and program oversight
  • Strong organizational and time‑management abilities

Nice To Haves

  • Bachelor’s Degree in a Human Services field

Responsibilities

  • Overseeing the health, safety, and well‑being of individuals served while communicating needs with staff and team members.
  • Leading staff in delivering community‑based activities and ensuring alignment with each Individual Support Plan.
  • Completing, reviewing, and ensuring the accuracy of all documentation, including progress notes, billing, CareLogic schedules, and required reports.
  • Scheduling and facilitating team meetings, coordinating staff coverage, and maintaining required program ratios.
  • Providing direct support services, coaching staff in the field, and supporting the development and implementation of curriculum and program materials.
  • Coordinating transportation and maintaining vehicle records, training compliance, and related documentation.
  • Maintaining communication with families, agencies, and support team members while monitoring the quality of services and documentation.
  • Supporting staff through training, performance evaluation, schedule review, and collaboration with the Program Operations Supervisor.
