About The Position

Lantern Community Services helps New Yorkers impacted by homelessness move from surviving to thriving by pairing safe, supportive housing with wraparound services that build lasting stability and independence. Lantern was also recognized as a Top 2025 Workplace by City & State New York. The Hudson Bay is a brand-new, all-female shelter in the Bronx that houses 200 women aged 55 and older. We are seeking an experienced Team Leader to oversee case management services within the Hudson Bay shelter program. The Team Leader provides direct supervision and guidance to Case Managers, ensures high-quality service delivery, and supports clients in achieving housing stability and greater self-sufficiency. This role combines limited direct service responsibilities with strong leadership, oversight, and program coordination functions. The ideal candidate will have experience working with individuals experiencing homelessness and demonstrated supervisory experience in a social services setting.

Requirements

  • High School Diploma, HSE, or GED required
  • Minimum of five (5) years of experience in social services, housing, or case management required
  • At least two (2) years of supervisory experience in a social services setting required
  • Experience working with individuals experiencing homelessness, mental illness, substance use disorders, and/or low-income populations required
  • Strong leadership, organizational, and written and verbal communication skills
  • Proficiency in Microsoft Word and Excel
  • Ability to work evening and/or weekend shifts as needed

Nice To Haves

  • Associate’s or Bachelor’s degree in Human Services or a related field preferred
  • Knowledge of mental health, substance use, and homelessness service systems preferred
  • Bilingual English/Spanish preferred

Responsibilities

  • Conduct intake, orientation, and referral processes for new program participants and assign cases to Case Managers.
  • Maintain a small caseload (up to 10 clients), providing weekly or biweekly meetings and developing comprehensive Independent Living Plans.
  • Utilize Motivational Interviewing and Harm Reduction approaches to deliver client-centered support and promote successful transitions to independent living.
  • Provide ongoing supervision, guidance, and support to Case Managers to ensure adherence to program standards, best practices, and performance expectations.
  • Review and approve case documentation to ensure accuracy, quality, and compliance with agency and funder requirements.
  • Facilitate regular case reviews to evaluate client progress, address barriers, and implement actionable service strategies.
  • Monitor team caseload distribution to ensure equitable assignments and timely service delivery.
  • Ensure services are aligned with clients’ housing, employment, and independent living goals.
  • Train, mentor, and coach case management staff to promote professional growth and high-quality service delivery.
  • Serve as a liaison to community providers, maintaining strong partnerships with housing, benefits, healthcare, and employment resources.
  • Refer clients to appropriate community resources, including SNAP, SSI/SSDI, public assistance, healthcare, employment, and educational services.
  • Advocate on behalf of clients with public agencies, landlords, and other stakeholders to reduce barriers to housing and services.
  • Maintain thorough documentation of client interactions, referrals, and service coordination activities; participate in client escorts and discharge planning as needed.
  • Participate in housing meetings, case conferences, staff meetings, supervision, and required trainings.
  • Perform other related duties as assigned.
