About The Position

The Team Lead will oversee the data entry and documentation of Case Manager caseloads, attend triage and hearing appointments with Department of Health and Human Services partners, and assist with day-to-day questions and guidance related to policy and procedures. Requirements

  • Experience working with FIP recipients/PATH participants and/or other workforce programs
  • Excellent leadership and communication skills, both written and verbal
  • Ability to maintain flexibility as needs of contract require
  • Proficient in Microsoft Office applications

Nice To Haves

  • Customer service experience preferred
  • Bachelor’s degree in business, education, human services, or related area is preferred

Responsibilities

  • Help to ensure that positive outcomes and program goals are met as required by contract
  • Supervise activities and conduct performance evaluations for case management staff
  • Model appropriate coaching and case management strategies
  • Perform case reviews to ensure that required documentation is maintained, staff are adhering to program policies, and participants are complying with their participation requirements
  • Review and approve participant requests for supportive services
  • Assist with participant case management tasks as needed
  • Document all participant contacts in case notes
  • Act as liaison and maintain collaborative relationships with local Dept. of Health and Human Services (DHHS) partners
  • Review Work Participation Rate (WPR) and other performance reports weekly and work with staff to address performance issues
  • Maintain a thorough knowledge of PATH policies and procedures
  • Coordinate participant transfers and other case changes with DHHS and other partners
