About The Position

A Team Lead provides oversight and supervision to staff and is responsible for coordinating comprehensive services for individuals their program supports. The Team Lead works closely with program leadership, as well as programmatic, community inclusion/employment specialists to ensure that each individual supported is empowered to live without limits including but not limited to the workplace and larger community.

Requirements

  • Valid Driver's License.
  • Bachelor's Degree in a Human Services field, including but not limited to sociology, special education, rehabilitation, nursing, counseling, and psychology OR
  • An associate degree with three (3) years' experience working to support individuals with intellectual disabilities OR
  • A High School Diploma with five (5) years' experience.

  • Effective Communication: Ability to clearly and respectfully communicate with residents, families, staff, and external agencies. Demonstrates active listening and ensures transparency in all interactions.
  • Ethical Practice: Upholds confidentiality, integrity, and ethical standards in all interactions and decisions, ensuring the rights and dignity of residents are prioritized.
  • Maintain and abides by OSHA requirements and guidelines
  • Observes all Fire and Safety Policies and Procedures
  • Observes all Fiscal/Office/ HR Personnel Policies and Procedures
  • Observes all HIPAA Policies and Procedures and maintains confidentiality
  • Attends all required and scheduled trainings and meetings
  • Always maintain a professional demeanor and exercise good judgement in all areas of employment duties

  • MUST HAVE VALID DRIVING LICENSE AND VEHICLE TO DRIVE EVERYDAY

Responsibilities

  • Provides operational support and oversight for community inclusion events.
  • Organizes, schedules, and distributes work among assigned personnel.
  • Keeps personnel informed of pertinent policies and procedures affecting the program and/or their jobs.
  • Administers policies and procedures.
  • Maintain direct service case load.
  • Direct care during community outings and/or vending events.
  • Maintains positive communication and relationships with family members, guardians, staff, and Regional Office personnel about policies, issues, and/or concerns.
  • Actively participates in the person-centered planning process, including but not limited to: 
  • Attending all planning meetings
  • Providing input
  • Coordinating the development of training plans
  • Ensuring all necessary documentation is completed
  • 1st reviewer of service notes 
  • Monitoring for the implementation of the plan
  • Completing monthly reviews
  • Prepare and submit applicable reports and documentation related to individuals supported by their program.
  • Ensure compliance with all regulatory bodies
  • Implement and complete continuous quality improvement activities
  • Other duties as assigned
