About The Position

Are you ready to transition from direct clinical work into a leadership role? As a Team Lead at Telecare’s AgeWise program, you will mentor a multidisciplinary team dedicated to helping older adults (59+) rediscover their health, hopes, and dreams. This is an entry-level management position designed for clinicians who want to sharpen their supervisory skills while remaining deeply rooted in community-based care. Schedule: Monday – Friday | 8:00 AM – 4:30 PM (Includes a structured, predictable on-call rotation) Compensation: $73,000 – $90,000 (Starting pay commensurate with experience)

Requirements

  • Master’s Degree and one (1) year of experience in mental health setting
  • One (1) year supervisory experience required or supervisory training within six (6) months of employment
  • Licensed eligible or waivered mental health professional as confirmed by State regulatory/licensing board (e.g., ASW, IMFT, or PCCI)
  • Sensitivity to multi-cultural populations and issues
  • Must be at least 18 years of age
  • Must be CPR, Crisis Prevention Institute (CPI), and First Aid certified on date of employment or within 60 days of employment and maintain current certification throughout employment
  • All opportunities at Telecare are contingent upon successful completion and receipt of acceptable results of the applicable post-offer physical examination, 2-step PPD test for tuberculosis, acceptable criminal background clearances, excluded party sanctions, and degree or license verification. If the position requires driving, valid driver’s license, a motor vehicle clearance and proof of auto insurance is required at time of employment and must be maintained throughout employment. Additional regulatory, contractual or local requirements may apply
  • Valid and current driver’s license, and personal vehicle insurance with your name listed as a driver.
  • Willingness to use your personal vehicle to drive clients to appointments and groups, etc. (weekly mileage reimbursement at the IRS rate)

Responsibilities

  • Mentor & Lead: Supervise a passionate team of providers, ensuring they have the support and resources needed to deliver excellent care.
  • Coordinate Care: Act as the bridge between multidisciplinary teams and outside agencies to ensure no member falls through the cracks.
  • Advocate: Work within our Strengths-Based Case Management model to help seniors navigate housing, medical care, and psychiatric support so they can live independently and with dignity.
