About The Position

The LPN Transition Liaison will assist the RN Transition Liaison in their daily role and processes to organize and facilitate thorough discharge plans to Home Health for the patients. The LPN Transition Liaison will be responsible for completing any clinical assessment for patients requiring Home Health. The LPN Transition Liaison will be responsible for assisting the Home Health DME tech by ordering and delivering of DME to patient’s rooms to facilitate a safe and timely discharge. Extra duties include, but are not limited: Assisting Home Health staff with completion of re hospitalization assessment. Interviewing patients, family members, and/or caretakers as needed to complete assessments. Maintaining open communication with care team regarding approval/denial of Home Health assessments. Maintaining proficiency in Home Health requirements based on Home Health regulatory and procedural processes. Assists with the arrangement and follow-up with community resources as needed. LPN Transition Liaison will assist in providing a seamless transition utilizing the entire continuum of care and helping to facilitate patient’s movement to Home Health.

Requirements

  • Graduate of an accredited school of nursing required.
  • Two years bedside experience required.
  • Two years Case Management preferred.
  • Licensed as a Licensed Practical Nurse in the states of Virginia and West Virginia required.
  • BLS Certification (Basic Life Support) - American Heart 'Healthcare Provider' (HCP) - AHA approved required.
