We are searching for a talented Team Lead-SNF Bed Placement-RN to be part of our community-focused and care-driven team. Requirements

• Current licensure by Board of Registration in Nursing (RN) is required.
• One to three years of nursing experience is required.
• At least two years of experience in home health or inpatient case management is required.
• Preferable experience in an administrative role.
• Must possess and demonstrate highly effective interpersonal, customer service, and communication skills.
• Requires proficient computer skills, including experience with various software, database, and electronic records programs.
• Must be fully vaccinated against seasonal Influenza and the COVID-19 virus or be exempt from the requirement for medical or personal reasons.

Responsibilities

• Communicate with Skilled Nursing Facilities (SNF) Owners regarding the overall strategy of Southcoast Hospitals.
• Coordinate all data and manage relationships with preferred SNFs and the SNF bed placement team.
• Lead meetings with Patient Financial Services and Case Management, documenting outcomes of productivity each month to meet KPIs.
• Manage the relationship with the SNF and communicate with their owners or Directors on monthly data and patient KPIs for quality reporting.
