About The Position

Description and Primary Purpose of Position: The Team Lead position is responsible for providing administrative and clinical supervision, coordination, consultation, and facilitation of Intensive Case Management teams provide intensive comprehensive treatment/services to individuals with severe and persistent mental illness in the community where they live.

Responsibilities

• Lead Treatment Team at least 1 day per week
• Maintain a case load of at least 5 Consumers.
• Review staff caseload and schedule to ensure they can maintain their required unit count.
• Supervise all members of the team.
• Assist with the Referrals.
• Ensure that Consumers are scheduled for at least 4 visits per week.
• Ensure all Failed Activities are completed within 24hrs by staff.
• Participate in ASO Audit
• Participate in CARF Review
