Since 1985, BHCHP's mission has been to ensure unconditionally equitable and dignified access to the highest quality health care for all individuals and families experiencing homelessness in greater Boston. Over 10,000 homeless individuals are cared for by Boston Health Care for the Homeless Program each year. We are committed to ensuring that every one of these individuals has access to comprehensive health care, from preventative dental care to cancer treatment. Our clinicians, case managers, and behavioral health professionals work in more than 30 locations to serve some of our community's most vulnerable—and most resilient—citizens. Yet true healing often requires more than clinical care alone—it takes connection, coordination, and compassion beyond the exam room. To meet this critical need, we are excited to introduce a new, non-clinical, two-part role on our Street Team. This position combines complex case management within the Behavioral Health Community Partners (BH CP) care management program with leadership responsibilities as a Senior Case Manager. In this role, you will collaborate closely with a multidisciplinary team to support highly vulnerable, predominantly unsheltered individuals experiencing mental illness and/or substance use disorders. You will play a key role in connecting individuals to behavioral health services, primary care, and essential social determinants of health supports—helping them navigate systems, build stability, and move toward improved health and well-being.
