About The Position

TCU Nurse 3:00 PM-11:00 PM
Pawtucket Falls is seeking a compassionate and skilled Registered Nurse (RN) or Licensed Practical Nurse (LPN) to join our Transitional Care Unit (TCU) team. The TCU Nurse is responsible for providing high-quality nursing care to patients transitioning from hospital to home or long-term care, ensuring a smooth and safe recovery process.

Requirements

• Current RN or LPN license in the State of RI
• Graduate of an accredited nursing program
• Previous experience in skilled nursing, rehab, or acute care preferred
• Strong clinical, communication, and organizational skills
• Ability to work effectively in a fast-paced environment
• Compassionate and patient-centered approach to care

• Previous experience in skilled nursing, rehab, or acute care preferred

Responsibilities

• Provide direct patient care in accordance with individualized care plans
• Administer medications and treatments as prescribed
• Monitor and assess patient conditions and report changes promptly
• Collaborate with interdisciplinary teams to support patient recovery
• Educate patients and families on care plans, medications, and discharge instructions
• Maintain accurate and timely documentation in compliance with state and federal regulations
