About The Position

The TCHATT Clinical Referral Specialist is responsible for the clinical evaluation and coordination of referrals received by the department. This role involves working closely with school employees, TCHATT providers, and leaders to ensure that referrals are clinically appropriate for the TCHATT program, facilitating a smooth and efficient coordination of care for patients.

Requirements

  • Master's Degree in Social Work, Counseling, or a related field.
  • Current Licensed Master Social Worker (LMSW), Licensed Clinical Social Worker (LCSW), or Licensed Professional Counselor (LPC) licensure from the Texas State Board of Social Work Examiners.
  • Current CPR certification, which must be maintained throughout employment.

Nice To Haves

  • 2+ years of professional social work experience in an outpatient behavioral health care setting.
  • Experience in coordinating patient acceptance and continuity of care planning.
  • Experience conducting initial screenings and evaluations of patients and families.
  • Experience providing or making referrals for individual, family, and group psychotherapy.
  • Experience promoting patient navigation services.

Responsibilities

  • Monitor electronic systems for patient referrals from schools.
  • Maintain accurate tracking of referral status and processes.
  • Gather pertinent patient information from various resources such as schools, patients, and family members.
  • Make clinical decisions regarding the fit of referrals for the TCHATT program.
  • Coordinate referrals and consult with other service providers and community resources as appropriate.
  • Coordinate referral approval with providers.
  • Work with schools to gain additional clinical insight into referrals or discuss reasons for declining referrals, providing appropriate resources as necessary.
  • Coordinate patient care with team members and providers for scheduling of approved patients, including assigning referrals to therapists and case managers as needed.
  • Chart assigned patients per operating procedure and maintain other records as assigned.
  • Maintain current knowledge of community resources and issues affecting the populations served.
  • Exhibit professional growth and development.
  • Assume responsibility for psycho-educational services or other responsibilities as assigned.
