About The Position

GENERAL RESPONSIBILITIES: The TCM Specialist works in one of the service providing programs of CBH. The primary function of the TCM Specialist is to provide medically necessary services as set forth in the consumer’s treatment plan. The TCM Specialist will be responsible for helping individuals access needed medical, social, educational, and other services. The TCM Specialist will be responsible for monitoring treatment plans and progress notes to assure the plan is working and will report findings to the Program Supervisor on a monthly basis and will make changes as indicated. ESSENTIAL JOB FUNCTIONS: CBH expects its employees to carry out essential job duties based on our CARE values model

Requirements

  • A BA/BS degree or be equivalently qualified by work experience or a combination of work experience in the human services field and education, with one year of experience substituting for one year of education.
  • Staff member shall possess demonstrated interpersonal skills, ability to work with the mentally ill, ability to respond effectively in a wide variety of human service situations.
  • Must be at least 21 years of age and have a current valid driver’s license.
  • Must successfully complete Case Management certification as stipulated by the State of Kansas within six months of employment and maintain Case Management Certification thereafter.
  • Must pass KBI, SRS, child abuse check, adult abuse registry, and motor vehicle screens.
  • Case Manager will obtain Strengths Model Case Management certification within six (6) months of employment and will obtain Advanced Case Management training annually.
  • This position requires a current, valid Kansas Driver’s License.
  • Will obtain CPR and First Aid Training and renew training annually.
  • Will attend any other training and certifications directly related to this position.
  • The employee is regularly required to talk, see and hear.
  • The employee is regularly required to stand, walk, sit, use hand and finger, use fine motor skills, handle or feel objects, tools or controls; and reach with hands and arms.
  • The employee frequently lifts and/or moves up to twenty pounds.
  • Specific vision abilities required by this job include close vision, the ability to adjust focus, and the ability to see to drive.
  • Specific hearing abilities required by this job include the ability to hear what is considered to be normal conversation tone.

Nice To Haves

  • Experience working with persons diagnosed with a Severe and Persistent Mental Illness is preferred.
