The Targeted Case Manager provides Rule 79 Case Management services to eligible persons primarily referred directly from Hennepin County or self-referral, who have been diagnosed with a serious and persistent mental illness. TCM services assist with access to medical, social, and other services. Targeted Case Managers ensure continuity of care coordination and monitoring of services, discharge planning, housing support and intensive goal planning through the development of the Individual Community Support Plan. The Targeted Case Manager would utilize a person-centered approach to work in partnership with persons served with the goal of improved functioning through acquisition of necessary supports, skill development and continuity of services. Here at SpringPath we provide services to people-served who are experiencing significant mental health/psychiatric symptoms, who are demonstrating significant functional impairments, are often experiencing complex challenges with substance use, poverty, being unhoused and often intersect with the criminal justice system.
