About The Position
LifeStream is a behavioral health and social services organization that provides high-quality treatment, education, care management, rehabilitation, child welfare, primary care and homeless services to children, adolescents, and adults. Located in Central Florida, LifeStream primarily serves the residents of Lake, Citrus and Sumter Counties, with additional programs located in Orange, Marion, Hernando counties. The Targeted Case Manager II function as an advocate and support person to those individuals assigned to their assigned program and links them to appropriate services and monitors the progress or lack of progress by the individual and his or her family. The Targeted Case Manager II will work closely with the individual, the individual's family, LifeStream staff, and other community organizations to meet their needs, achieve their goals, and optimize their level of independence.
