About The Position

- The Targeted Case Manager I at LIFESTREAM BEHAVIORAL CENTER is responsible for providing comprehensive case management services to individuals with mental health and behavioral challenges. The role involves assessing client needs, developing individualized care plans, and coordinating services to ensure clients receive the necessary support for their well-being and recovery.

Requirements

  • Bachelor's degree in Social Work, Psychology, Counseling, or a related field from an accredited institution.
  • Minimum of one year of experience in case management or a related field, preferably within a behavioral health or social services setting.
  • Experience working with diverse populations and understanding of cultural competency.
  • Familiarity with community resources and services available for individuals with behavioral health needs.
  • Strong organizational and time management skills, with the ability to manage multiple cases efficiently.
  • Excellent communication skills, both verbal and written, with the ability to interact effectively with clients, families, and professionals.
  • Ability to assess client needs and develop comprehensive service plans.
  • Proficiency in using computer systems and software for case documentation and data management.
  • Demonstrated ability to work independently as well as part of a multidisciplinary team.
  • Strong problem-solving skills and the ability to handle crisis situations calmly and effectively.
  • Knowledge of relevant laws, regulations, and ethical standards in case management and behavioral health services.

Responsibilities

  • Conduct thorough assessments of clients' needs and strengths to develop personalized care plans.
  • Coordinate and facilitate access to necessary services, including medical, educational, and social support.
  • Monitor and evaluate client progress, adjusting care plans as needed to ensure effective support.
  • Advocate for clients' needs and rights, ensuring they receive appropriate resources and services.
  • Maintain accurate and up-to-date client records and documentation in compliance with organizational and legal standards.
  • Collaborate with a multidisciplinary team of professionals to provide holistic care and support to clients.
  • Provide crisis intervention and support to clients and their families as needed.
  • Educate clients and their families about available resources and support systems.
  • Participate in ongoing training and professional development to stay current with best practices in case management.
  • Adhere to all organizational policies and procedures, ensuring ethical and professional conduct at all times.
