About The Position

Post Release Community Supervision (PRCS), in collaboration with the Los Angeles County Probation Department, offers a range of services to individuals across the county, facilitated through various housing facilities including Transitional Housing, Reentry Housing, and specialized accommodations for parents with children, board and care, among others. Our reentry planning team involves Case Managers, System Navigators, and Service Coordinators. Our team provides support services including job training and permanent housing move-in costs, facilitated through Probation Department referrals. Our community-based programs foster partnerships with local health providers, housing advocates, employment resources like trade unions’ apprenticeships, and human services agencies. This comprehensive approach aims to meet the diverse needs of participants and their families, fostering resilience and reducing recidivism risks. The Systems Navigator provides comprehensive case management and advocacy services for individuals under the Los Angeles County Department of Probation’s Post Release Community Supervision (PRCS) program. This in-person role involves direct client engagement, needs assessment, service coordination, and support with reentry efforts. As this position requires regular travel throughout the County to meet with clients and partners, reliable transportation and a valid California driver’s license are required.

Requirements

  • High school diploma or equivalent plus two years of case management experience or Bachelor’s degree in human services field.
  • Valid First Aid and CPR certification or ability to obtain within 30 days of hire.
  • Valid California Driver’s License and access to registered and insured transportation.
  • Two years’ case management experience or behavioral health work experience preferred.
  • Experience in the human service field and demonstrated expertise in substance abuse treatment, relapse prevention, and recovery.

Responsibilities

  • Conduct telephonic and face-to-face assessments of client’s needs.
  • Assist clients in identify eligible public assistance programs.
  • Help clients complete necessary paperwork for public assistance.
  • Assist in obtaining essential documents (birth records, identification, and Social Security cards).
  • Manage clients throughout the case management process (from intake to benefit acquisition and housing).
  • Identify and procure ancillary services for clients.
  • Process auxiliary fund requests and refer to housing resources.
  • Ensure continuous communication with LA County Probation personnel.
  • Drive and transport clients or necessary materials as assigned.
  • Enter data accurately when creating or updating progress notes, case management plans, and other client-related documentation.
  • Monitor quality assurance of client documentation and services.
  • Ensure that subcontractor client files contain complete necessary documents.
  • Ensure confidentiality of client data (both incoming and outgoing).
  • Generate reports on client outcomes and services provided.
  • File and maintain project documents and records.
  • Complete all assigned training in a timely manner.
  • Attend and participate in meetings and training opportunities.
  • Read and respond to emails/messages in a timely manner.
