Trillium Health Resources has a career opening for a System of Care Coordinator to join our team! The System of Care (SOC) Coordinator is responsible for providing overall coordination and oversight of developing and supporting a system of care for young people and families receiving child mental health and substance use services for assigned counties in the Trillium Catchment Area. This position ensures all program requirements are met and maintained. This position acts as the primary contact for other systems in the catchment area who work in partnership to support the identified population. In addition, this position will represent Trillium at public meetings related to any and all services and populations covered by Trillium; specifically, the Community Collaborative Meetings and Child MDT meetings. This position will assist with program development, consultation, resource management, coordination of other services, and agencies to best meet the needs of the persons served and also attend multi-disciplinary teams (MDTs) to assist the child and family care planning.
