The System Navigator is responsible for (health care navigation, medical payment source and other intensive social service needs, and communicating with clients in need of health and medical resources. This position works in collaboration with county service providers (i.e., health department, Medicaid office) to assist clients in securing needed services. The System Navigator effectively promotes program goals and communicates accurate information about the Healthy Beginnings program. The System Navigator is responsible for providing safe sleep services and education to caregivers of children under the age of 13 months in Palm Beach County. This position will provide services in assigned field location(s) and may require scheduling flexibility in order to accommodate client schedules. Here’s what the job involves: Contact and engage families upon referral, conducting home visits or community-based meetings to provide support. Assess client needs, using bilingual communication skills when required, and help families understand available health and payor resources. Assist clients in navigating county programs—including health, financial, and transportation services—and help remove barriers to accessing care. Provide hands-on support such as reading, translating, and completing medically related forms to ensure clients can initiate needed services. Serve as a liaison between participants and community partners to coordinate care and streamline service access. Maintain accurate and up-to-date client records, documentation, and data entry across required systems. Communicate regularly with the supervisor regarding schedules, client progress, and program outcomes. Participate in staff meetings and supervision while ensuring compliance with program goals, outcome measures, and reporting requirements.
