About The Position

The Sweet Dreams Home Health Aide (HHA) position at Bristol Hospice involves providing compassionate care to hospice patients and their families. The HHA plays a critical role in the Interdisciplinary Group Team, working under the supervision of a Registered Nurse to meet patients' personal needs and promote comfort. This role emphasizes a family-centered approach to hospice services, ensuring that all patients and families are treated with compassion, respect, and dignity.

Requirements

  • Home Health Aide certification required per state regulations.
  • Preferred to have one (1) year experience as an Aide in community health/hospice or medical/surgical experience.
  • Must have completed a minimum of 75 hours of classroom and supervised practical training.
  • Licensed driver with an automobile that is insured in accordance with organization/state requirements and is in good working order.
  • Willingness to submit to a criminal background check prior to hire and annually thereafter.
  • Current CPR certification.

  • Understanding of hospice philosophy and comfort in providing specialized care to the terminally ill.
  • Ability to work autonomously with little direct supervision.
  • Strong organizational skills.
  • Understanding of plan of care documentation.

Responsibilities

  • Spend quality one-on-one time with patients and their families to support everyday needs.
  • Care for hospice patients by providing baths, back rubs, oral hygiene, shampoos, and changing bed linens as needed.
  • Provide bedtime care including personal hygiene regimens as required.
  • Provide aromatherapy and gentle touch.
  • Perform personal care needs after receiving a patient's assignment and attending to requests promptly.
  • Record important health information including temperature, pulse, respiration, and blood pressure.
  • Assess and report patient condition and any significant changes to the Case Manager.
  • Assist patients with ambulation and exercise.
  • Maintain the patient's living area in a clean and orderly manner, as assigned.
  • Adhere to the organization's documentation and care procedures and standards of personal and professional conduct.
