About The Position
SWAN REHAB is seeking a dedicated and experienced Licensed Clinical Social Worker (LCSW) or Licensed Master Social Worker (LMSW) to join our team in Phoenix, Arizona. This full-time position is integral to our mission of providing exceptional neuro rehabilitation services to individuals recovering from traumatic brain injuries, spinal cord injuries, and other acute diagnoses. As a social worker in our program, you will play a crucial role in delivering therapeutic interventions and resource recommendations tailored to meet the unique goals of each patient. Your expertise will help facilitate their recovery journey, ensuring they receive the support they need to regain their independence and improve their quality of life. In this role, you will have the opportunity to work collaboratively with a multidisciplinary team, valuing professional autonomy while also emphasizing teamwork and communication. You will conduct comprehensive evaluations to assess the social, emotional, and support needs of patients and their families, setting measurable objectives in conjunction with the rehab team. Your insights will assist physicians and other health team members in understanding the significant social and emotional factors that impact patients' health problems. You will also provide counseling to patients and families, helping them cope with the emotional responses associated with their illnesses. Your responsibilities will extend to care coordination activities, where you will act as a resource for other health team members in identifying and resolving patient needs. Additionally, you will identify and assist patients and their families in utilizing appropriate community resources to achieve their objectives.
