About The Position

The Survivor Care Transitional Services Specialist at Love146 is a full-time role focused on supporting participants aged 16 and above in the Connecticut Survivor Care Program. The position aims to assist these individuals in achieving their goals as they transition into adulthood, with a strong emphasis on secondary and post-secondary education, employment, housing, and related case management services. The specialist will implement transitional services programming with excellence, ensuring that participants receive the necessary support to succeed in their personal and professional endeavors.

Requirements

  • Bachelor's degree in a related field such as human resources, social work, business, or education preferred; or Associate's degree with 2-3 years of related work experience.
  • Prior experience in transitional services such as workforce development or housing placement.
  • Experience working directly with adolescents and young adults, preferably with high-risk populations.
  • Valid driver's license with a clean driving record.

  • Experience providing community-based services preferred.

Responsibilities

  • Implement Love146's Survivor Care program's Transitional Services programming services with a standard of excellence.
  • Help participants to identify individual Care Plan goals and measurable objectives for obtaining those goals.
  • Provide direct services to program participants to help them achieve their Care Plan goals, including resume building, job searches, and housing assistance.
  • Liaise between program participants and various resources such as educational institutions, employers, and community agencies.
  • Track participant's progress in obtaining their Care Plan goals and modify services as needed.
  • Identify and maintain relationships with post-secondary education institutions and service providers to support participant success.
  • Document and track participant data for program monitoring and evaluation.
  • Collaborate with internal departments and participate in Love146 meetings to promote a cooperative culture.
