As the Survey Administrator, you will play a pivotal role in administering and conducting outreach to Members of Fountain House ensuring that our work leads to systemic change and sustainable support for people living with serious mental illness. Must have New York Peer Specialist Certification $24.04 per hour Full-time inhouse (Hell's Kitchen) Monday to Friday 9am to 5pm 1 day per month in Harlem & Bronx

Requirements

  • Must have New York Peer Specialist Certification
  • Knowledge of HIPAA privacy and practices
  • Experience in administrative support, data entry, customer service, or related field.
  • Proficiency with basic office software (e.g., survey monkey, Microsoft Office, Excel and Air Table).
  • Familiarity with mental health clubhouse settings
  • Minimum of two (2) years of work experience providing psychosocial rehabilitation services with individuals experiencing serious mental illness
  • Bilingual in Spanish required

Responsibilities

  • Collaborate, as a team of 3 Peer Specialists, in outreach efforts to members of the Fountain House Midtown Clubhouse.
  • Make reach-out calls to set up appointments to complete three surveys in adherence to our DOHMH Clubhouse contract
  • Assist the members, as needed, in completing the surveys and then submitting them to DOHMH.
  • Reading questions aloud either over the phone or in-person.
  • Transcribing member responses into the survey monkey.
  • Assisting with understanding survey content as needed.
  • Work collaboratively with the unit social practitioner to connect with the member.
  • Work as a team to ensure timely submission of completed surveys in accordance with DOHMH guidelines
  • Ensure accurate records of outreach efforts, scheduled appointments, and survey completion status
  • Uphold confidentiality, sensitivity, and respect when working with all members.
