We’re seeking a motivated individual who is dependable, adaptable, and interested in building a long-term career with us. Purpose: Supervises Supports Coordinators within the unit Trains, mentors and evaluates the Supports Coordinators of their team. Supervises the Supports Coordinators in the daily activities and schedules related to their individual caseload assignments. Assures that monitorings, IM4Q responses, service notes, and ISPs are completed within necessary time frames. Reviews and submits consumer annual Individual Service Plans in HCSIS. Facilitates the implementation and evaluates the effectiveness of the plans for the Supports Coordinators on their team. Oversees the maintenance of supports coordinators caseloads records & ensures paperwork is consistent with agency, regulatory, HCSIS & funding requirements. Reviews Daily Activity Logs for consistency with HCSIS. Ensures that HCSIS service notes are entered within two days of the service provision. Attends County meetings/committees as a representative of the Unit, as needed. Establishes and maintains consumer friendly environment in the office. Creates spirit of customer service throughout all Supports Coordination Activities. Monitors staff schedules, attendance and approves time-off. Tracks in-house training for Supports Coordinators on team and ensure that their training is up-to-date. Continually develops team knowledge & cooperation. Identifies and compiles reported gaps in the service delivery system and reports them to Management Team. Performs necessary functions in the state HCSIS system. Perform other duties and responsibilities as they relate to the overall enhancement of program service delivery.
