The Supportive Housing Case Manager position at Danville-Pittsylvania Community Services involves providing assessment, linkage, and advocacy services to adults with serious mental illness, co-occurring disorders, and/or substance use disorders. The role requires working in vivo at a level of intensity necessary to promote individual empowerment and reduce the need for inpatient treatment, as well as to promote family stabilization. The case manager will develop and implement individual service plans, engage in pre-discharge planning at state and private psychiatric facilities, and provide support services to caregivers. Additionally, the case manager will participate as a member of the Intensive Community Services team, complete required documentation as prescribed by licensure, Medicaid, managed care groups, and other third-party payers, and comply with all regulations applicable to program services. Specialized services will be provided to meet a variety of individual needs, including housing, education, risk assessment, and prevention of infectious disease, with sessions conducted in individual, group, and family settings, both on-site and in the community.

  • Bachelor's degree in psychology, social work, vocational rehabilitation, counseling, or other VA DMAS approved Human Services degree.
  • Certified psychiatric rehabilitation practitioner in Virginia.
  • Valid driver's license and safe driving record.
  • Fingerprints, State and FBI criminal record reports, drug test, and central registry (CANIS) report will be required upon request.
