General Position Summary: The Supported Services Specialist I provides direct support and internal case management for individuals with disabilities participation in Garten Services programs. This position supports individuals in developing, maintaining, and strengthening skills related to daily living, community inclusion, vocational readiness, and employment-related goals in alignment with their Individual Support Plan (ISP), Vocational Rehabilitation (VR), or other funding requirements. This role is responsible for maintaining accurate documentation, coordinating with internal and external partners, and delivering direct support services in program, community and employment-related settings as assigned. The Supported Services Specialist I models Garten's Mission, Vision, and Values and acts as a positive role model to individuals supported, coworkers, and community partners.

  • Minimum of 1-2 years' experience working with or providing services to individuals with disabilities or An Associate degree or higher in rehabilitation, social work, social sciences, communication, or a related field may substitute for experience
  • High school diploma or equivalent required
  • Ability to stand, walk, bend, twist, kneel, reach, and grasp during a normal workday
  • Ability to assist individuals with mobility needs, including wheelchair support
  • Ability to push up to 100 pounds and carry up to 10 pounds occasionally
  • Ability to lift up to 50-100 pounds occasionally with or without assistance

Responsibilities

  • Provides direct support to individuals with disabilities in program, community, and employment-related environments in accordance with individual plans and goals.
  • Maintains internal case management for assigned caseload, including documentation such as progress notes, case notes, safety plans, billing records, and other required records.
  • Implements, documents, tracks, and reports progress toward ISP, VR, or program goals.
  • Assesses individual skills, interests, strengths, and support needs to inform service delivery.
  • Provides skill-building, coaching, and motivational support to promote independence, community participation, and vocational readiness.
  • Responds in a timely manner to individual support needs, including on-the-job or int-the-moment support when required.
  • Acts as a liaison with funding sources, community partners, employers (as assigned), residential providers, families, and circles of support.
  • Participates in and may facilitate Individual Support Plan meetings, team meetings, and interdisciplinary team (IDT) meetings as assigned.
  • Ensures all recordkeeping for assigned caseload is current, accurate, and completed within required timelines.
  • Ensures confidentiality of individuals receiving services.
  • Ensures compliance with Garten policies, safety standards, and applicable regulations.
  • Administers medications and maintains Medication Administration Records (MARs) when trained and authorized.
  • Provides personal care assistance as needed, including restroom support and assistance with mobility.
  • Assists with transportation of individuals as required, including driving agency or personal vehicles.
  • Acts as a positive role model and contributes to a safe, respectful, and inclusive service environment.
  • Communicates regularly with team members to ensure coordinated and consistent service delivery.
  • Assists with planning and supporting program, recreational, or community-based activities.
  • Maintains required trainings, certifications, and continuing education.
  • Assists with inventory and maintenance of program supplies and materials.
  • Provides feedback and input on service improvement, workflows, and processes.
  • Participates in committees, meetings, or workgroups as assigned.
  • Performs other related duties as assigned.
