About The Position

As a Supported Living Assistant Manager at Pathfinder, you play a key leadership role in ensuring individuals receive safe, consistent, and person-centered supports in their homes. By supervising teams, coordinating care, and supporting daily operations, you help create stable environments where people can thrive and live meaningful, independent lives. As a Supported Living Assistant Manager at Pathfinder, You Will: Leadership & Staff Support Provide direct oversight and support to Direct Support Professionals (DSPs) across assigned residential sites Serve as the primary point of contact for DSP scheduling needs and coverage concerns Lead team meetings to reinforce expectations, communication, and person-centered practices Monitor staff training compliance and model ethical, effective leadership Individualized Supports & Care Coordination Support the implementation of individualized support plans (PCISPs) in collaboration with the Supported Living Manager Coordinate medical appointments, day program participation, and transportation needs Participate in PCISP, quarterly, and leadership meetings as assigned Operations, Compliance & Documentation Ensure homes meet safety, health, and quality standards at all times Maintain accurate case notes, billing records, and Medicaid Waiver documentation Oversee medication administration, documentation, and refill processes Identify and report maintenance and operational needs promptly Communication & Collaboration Act as a liaison between families, guardians, DSPs, and leadership Communicate staffing or operational concerns to the Supported Living Manager in a timely manner Collaborate with leadership and on-call teams to ensure shift coverage when needed The Impact You’ll Make as a Supported Living Assistant Manager: Ensure individuals receive consistent, high-quality, person-centered supports Support and develop DSP teams through guidance, structure, and leadership Strengthen communication and continuity of care across residential services Advance Pathfinder’s mission by promoting dignity, safety, and empowerment Pathfinder’s mission is to strengthen communities by enabling people with disabilities or economic challenges to achieve autonomy, inclusion, and stability.

Requirements

  • At least 18 years of age.
  • Basic language, written, and computer skills necessary for communication and documentation.
  • Demonstrated ability to support people with disabilities.
  • Ability to work a flexible schedule if needed.
  • Crisis prevention, intervention, and teaching skills.
  • Valid driver’s license, insured vehicle, and safe driving record.
  • Ability to speak, hear, and read the English language.

Nice To Haves

  • Minimum of a high school diploma/GED.
  • Prefer 2 years experience in the field of Human Services.

Responsibilities
