Summary: Under the direction of the Director of Early Intervention Services and programmatic supervisor, the Individual Placement and Support Specialist provides evidence based supportive employment and education services.

Requirements

  • Bachelor's or Associate's degree from an accredited college or university in education, social work, human resources, healthcare, or disability related field of study; or high school diploma or GED and two years of experience in education, social work, human resources, healthcare, or disability related field of study.

Responsibilities

  • Trained in and utilizes Individualized Placement and Support Practice and Principles, (IPS), to a minimum caseload of 10 clients and a maximum caseload of 20 clients.
  • Maintains a minimum of 65% community time as defined by the IPS fidelity manual.
  • Meets with a minimum of 6 hiring managers per week as defined by the IPS fidelity manual.
  • Coordinates with area businesses on criteria for employment.
  • Explores possible job matches to the client’s skills and experience.
  • Provides mediation in the event of concerns with current employment.
  • Makes referrals to Vocational Rehabilitation to holistically coordinate care in promotion of the client meeting their employment goals.
  • Participates in the development of individual program plans and provides ongoing information regarding client progress to the service providers.
  • Participates in the development of crisis/safety plans as warranted.
  • Writes progress notes after each service contact in accordance with the Center’s procedure on Time Frames for Clinical Records and Related Documentation
  • Attends weekly IPS team supervision, weekly individual supervision, and weekly treatment team supervisions.
  • Works collaboratively with the OnTrack team, promoting strength based services, team cohesiveness, coverage for other team members when needed/appropriate, etc.
  • Is expected to have regular and predictable attendance.
