Established in 1975, Non-Profit Yakima Neighborhood Health Services serves patients in the areas of Behavioral Health, Family Dentistry, Internal Medicine, Women’s Health, Primary Care, Pediatrics, Vision Care and Pharmacy. Our mission is to improve the quality of life and equity in our communities by providing accessible and integrated health and social services, ending homelessness and offering unique learning opportunities for students of health professions. The Supported Employment Case Manager engages consumers and establishes trusting, collaborative relationships directed toward the goal of competitive employment in integrated job settings. This role involves assessing consumers’ vocational functioning, assisting with job development and job search activities, providing individualized and time-unlimited follow-along services to help consumers sustain employment, and offering education and support to employers. The Case Manager will also provide outreach services, meet regularly with team members to coordinate and integrate vocational services into physical and mental health treatment, draw up and update individual employment plans, and work in the community to engage and support consumers, family members, and employers.
