About The Position

The position involves monitoring service delivery for a caseload of program participants. The role is full-time and non-exempt, requiring adherence to various policies and procedures while ensuring timely completion of assessments and service plans. The individual will maintain contact with participants, assist them in obtaining community support, and ensure all documentation is accurately maintained and submitted according to established procedures. Additionally, the role includes attending mandatory meetings and trainings, responding to communications, and providing on-call support as needed.

Requirements

  • Bachelor’s degree in the Human Service Field
  • Must become certified in IHC and Plan of Care within the first three months of employment
  • Maintain valid driver’s license and vehicle insurance
  • Excellent writing and computer skills required

Responsibilities

  • Review referrals and assess program eligibility
  • Complete intake and initial assessment of services timely
  • Adhere to all of the OAAS policies, procedures and timelines
  • Develop and monitor service plans in accordance with regulatory guidelines
  • Maintain contact with program participants in accordance with regulatory guidelines
  • Assist with developing and monitoring monthly budgets, where applicable, for program participants
  • Assist program participants with obtaining community support, resources and services
  • Maintain required documentation (service logs, POCs, etc)
  • Submit service logs and billing documentation according to procedure
  • Attend all OAAS Mandatory meetings and trainings
  • Check and respond to all phone calls daily
  • Maintain training requirements
  • Report any significant concerns and critical incidents to supervisor/SIMS
  • Rotate on call 24 hour support to program participants
