We provide Support Coordination and Service Navigation for adults with Intellectual, Developmental, and Physical Disabilities, Autism Spectrum Disorder, and aging adults enrolled in the Department of Human Services Medicaid Waiver Program. As a Support Coordinator, you play a meaningful role in helping people access the services they need-from housing and medical care to community engagement and independence. Requirements

  • Bachelor's degree in social work, Psychology, Human Services, Education, or related field.
  • Strong organizational, communication, and advocacy skills.
  • Ability to work independently and travel (up to 50%) to meet participants in their homes and communities.
  • Valid driver's license, auto insurance, and reliable transportation.
  • Commitment to maintaining required certifications and state clearances.

Responsibilities

  • Conduct person‑centered assessments to understand each participant's needs, preferences, and goals.
  • Develop, implement, and update Individual Support Plans (ISP) to ensure participants receive appropriate services.
  • Provide advocacy and help participants make informed decisions about providers and care options.
  • Coordinate with medical professionals, behavioral health providers, family members, and community resources.
  • Monitor services to ensure safety, satisfaction, and progress toward independence.
  • Support individuals with disabilities and aging adults in accessing community resources.
  • Maintain accurate documentation and uphold confidentiality and compliance standards.
  • Participate in ongoing training and professional development.
