Requirements

  • Bachelor’s degree in social work (BSW) or equivalent human service degree.
  • Registered Nurse (licensed)
  • This position requires a valid driver’s license, a reliable vehicle, and proof of insurance.

Nice To Haves

  • Prior social services case management experience
  • Professional experience working with individuals with developmental or intellectual disabilities.
  • Knowledge of Medicaid-funded services, community based supports, and social services systems.

Responsibilities

  • Independently manage a professional caseload of individuals with developmental and intellectual disabilities within an assigned geographic area, exercising discretion and professional judgment in prioritizing cases, assessing service needs, and determining appropriate interventions.
  • Conduct comprehensive, in-depth assessments through interviews with individuals served, families, guardians, and service providers, and analyze information from multiple sources to identify strengths, needs, risks, service gaps, and eligibility considerations.
  • Exercise professional judgment to develop, implement, and regularly update individualized, person-centered support plans in collaboration with interdisciplinary teams, ensuring plans are responsive to individual goals, assessed needs, and changing circumstances.
  • Coordinate, authorize, and monitor services to ensure alignment with individualized support plans, funding eligibility criteria, and applicable regulatory requirements, making determinations regarding service scope, level, and continuation.
  • Participate as a professional member of interdisciplinary teams by contributing analysis, recommendations, and informed decision-making related to service levels, provider selection, and modifications to support plans.
  • Evaluate individual progress and service effectiveness through ongoing professional assessment and outcomes data, and recommend adjustments to services or supports to promote quality outcomes and continuity of care.
  • Prepare, review, and approve purchase-of-service authorizations and related documentation in accordance with program guidelines, funding requirements, and professional standards, ensuring appropriate use of resources.
  • Maintain responsibility for accurate, timely, and compliant case documentation, including assessments, support plans, progress summaries, and required reports, utilizing electronic case management systems in accordance with regulatory and agency standards.
  • Serve as a professional liaison with provider agencies, state partners, and community resources to ensure service quality, coordination, and continuity, addressing issues and resolving service concerns through professional judgment.
  • Represent the agency in meetings with service providers, community partners, and stakeholders, providing professional insight, recommendations, and case coordination expertise.
  • Apply professional ethics, confidentiality standards, and regulatory compliance requirements in all aspects of service coordination and decision-making.
  • Performs other related duties as assigned.
