About The Position
We are currently looking for a Full-Time Support Coordinator to support individuals with disabilities in Atlantic, Burlington, Middlesex and Monmouth county. The successful candidate will be responsible for assisting participants to gain access to needed programs, services and supports including but not limited to medical, social, educational and other services. The Support Coordinator in conjunction with the participant will also develop Individual Service Plans and maintain ongoing contact to assure services are being provided to the individual's satisfaction. This position is hybrid remote. Applicant will be expected to meet clients in various counties throughout NJ on a regular basis.
