The Support Coordinator provides targeted case management/support coordination activities for individuals diagnosed with Intellectual Disabilities or Developmental Disabilities (ID/DD). Works as part of the team to ensure individuals are linked with necessary services, supports, and resources. Ensures supports are coordinated amongst providers as needed to help individuals live and function optimally in the community.
