About The Position

The Support Coordinator provides targeted case management/support coordination activities for individuals diagnosed with Intellectual Disabilities or Developmental Disabilities (ID/DD). Works as part of the team to ensure individuals are linked with necessary services, supports, and resources. Ensures supports are coordinated amongst providers as needed to help individuals live and function optimally in the community.

Requirements

• High School Diploma is required.
• Bachelor's Degree in Education, Social Work, Psychology or Human Services related field is preferred.
• At least one year of experience in a setting working with individuals with mental illness, substance use disorders and/or intellectual/developmental disabilities is required.
• Registration as a QDDP is required. Must maintain QDDP status and all required trainings/CEUs.
