About the position

The Department for Persons with Disabilities (DPD) is one of the largest Catholic Charities Agencies providing care for adults with intellectual and developmental disabilities in the United States. We are looking for dedicated, caring and compassionate people to work in our Group Homes. The Support Coordinator will manage support coordination services for each participant, including individual discovery, ISP development, plan coordination and plan monitoring. This role requires effective communication with supervisors, participants and their families, other service providers, and DDD representatives. The job also involves a significant amount of written documentation and cooperation with the agency's administration and/or Division and DHS staff in any inspection or investigation.

Responsibilities

  • Manage support coordination services for each participant.
  • Conduct individual discovery and ISP development.
  • Coordinate and monitor support plans.
  • Communicate effectively with supervisors, participants, families, and service providers.
  • Document written reports and maintain records.
  • Cooperate with agency administration and staff during inspections or investigations.

Requirements

  • Bachelor's Degree or higher in any field.
  • 1 year of experience working with adult individuals (21 or older) with developmental disabilities.
  • Valid driver's license in the State of New Jersey.
  • 40-60 WPM typing skills.
  • Intermediate to advanced computer skills.
  • Strong organizational skills.
  • Ability to operate office equipment.

Nice-to-haves

  • Experience providing care coordination to different populations with some individuals having developmental disabilities.
  • Experience as a family caregiver of an adult with a developmental disability.
