About The Position

The Nurse Practitioner is responsible for delivering comprehensive and compassionate medical care to hospice patients, emphasizing symptom management, pain relief, and enhancing quality of life. This role involves collaboration with the Medical Director and the Interdisciplinary Team (IDT) to assess patients, develop care plans, prescribe medications, and provide education to patients and their families, ensuring coordinated care throughout the end-of-life journey.

Requirements

• Valid Nurse Practitioner license.
• Experience in hospice or palliative care.
• Strong assessment and diagnostic skills.
• Ability to work collaboratively in a team environment.
• Excellent communication and interpersonal skills.
