About The Position

The PACT Social Work Supervisor provides social work supervision for the Patient Aligned Care Team (PACT) social workers at the Baltimore VA Medical Center and provides direct social work services to Veterans who are not currently assigned to a VAMHCS primary care team or to BT PACT teams as needed. Responsibilities

  • Provides social work supervision for the Patient Aligned Care Team (PACT) social workers
  • Provides direct social work services to Veterans who are not currently assigned to a VAMHCS primary care team
  • Available to assist with in-person coverage for PACT social work at any VAMHCS site, as needed in times of staffing shortages
  • Assess and treat the psychosocial problems that impact on the ability of the veteran to make adjustments to their health care issues
  • Uses a variety of interventions in the amelioration of medical conditions
  • Supports the veteran in the community by securing resources and linking the veteran and family to those services
  • Ensures adequate daily social work coverage for the Baltimore PACT, including coverage for expected and unexpected leave
  • Trains new PACT social work staff; assists with cross coverage as needed for VAMHCS PACT social workers
  • Ensures timely completion of Primary Care Social Work consults for Baltimore PACT social workers works with Baltimore PACT social workers to meet urgent or emergent same-day patient needs when the assigned PACT social worker is unavailable
  • Provides social work services to patients who are not currently assigned to a primary care provider; and works with Veterans and their families and/or caregivers who have highly complex care needs
  • Establishes guidelines and performance expectations for staff
  • Regularly observes employees' performance and demonstrates, as well as conducts, work performance appraisals
  • Receives and handles informal complaints and grievances
  • Provides advice and counsel to employees related to work and administrative matters, coordinating with the Assistant Chief of Social Work, Ambulatory and Emergency Care Clinical Center
  • Reviews approves/ disapproves all leave requests as well as ensures all employees are trained and fully comply with all trainings and the provisions of the safety regulations
  • Demonstrates the ability to expand clinical knowledge in the profession
  • Provides consultation and guidance effectively to colleagues and supervisees, as role model effective social work practice skills, teach or provide orientation to less experienced social workers
  • Develop innovations in practice interventions, and provide clinical supervision for social work licensure or certification
  • Assigned to be administrative responsibility for clinical program development and is accountable for clinical program effectiveness and modification of service patterns
