This position directly supervises the social workers at the William Jennings Bryan Dorn VA Medical Center in Columbia, SC assigned to the Geriatric and Extended Care (GEC) Community Health Programs, including Community Adult Day Healthcare, Community Nursing Homes, and Veterans Directed Care. Develops policies and procedures for program operation and preparing reports and statistics for use at service, facility, VISN, and National reporting levels. Participates in CNH program interdisciplinary team meetings, provides insight into program operations, and recommends improvements. Provides direct clinical services through visitation to Veterans residing at VA-approved community nursing homes and community care network facilities. Communicates with family/caregivers and facility staff to address and resolve patient concerns. Provides input as needed on CNH program operations. Serves as a voting member of the CNH Oversight and Review Team. Manages and coordinates consults and referrals, and is accountable for clinical program effectiveness. Responsible for the clinical program development, and education/training of program operations to Social Work Service line and facility staff. Serves as team leader for social work-related issues impacting and responding to PATS-R, congressional, and White House inquiries. Provides direct clinical services and resolves problems that arise regarding patient care issues and requires specialized knowledge of issues related to community-based services. Expands clinical knowledge in the profession, provides consultation and guidance to colleagues, and serves as a role model for effective social work practice skills. Develops innovations in practice interventions and provides clinical supervision for social work licensure or certification. Assists in planning, establishing, and implementing policies and procedures. Develops individual or group goals and objectives as well as monitoring, operating, evaluating, coordinating, and overseeing the Social Work staff of the assigned programs. Provides supervision for subordinate staff to qualify for licensure. Ensures supervised employees are aware of and adhere to all current directives of VHA, and meet current JCAHO standards, state licensure standards, and VA-OIG expectations. Conducts ongoing intensive reviews and appraisals to ensure the accomplishment of objectives and adherence to standards. Supervises the assignment of work and review, as well as the administrative and personnel management functions related to assigned staff. Establishes guidelines and performance expectations for staff. Observes staff performance, and provides demonstration and conduct work performance critiques as necessary. Provides informal feedback and periodically evaluates employee performance. Works to resolve informal complaints and grievances. Develops work improvement plans and provides recommendations for personnel actions. Invokes disciplinary measures as appropriate to the authority delegated in this area. Reviews and approves or disapproves leave requests. Assures that subordinates are trained and fully comply with the provisions of any governing safety regulations. Supervises Social Work Students. Performs other duties as assigned.
