Requirements

• A current LPN license in the state of services.
• Experience working with individuals with intellectual and developmental disabilities preferred.
• A valid driver's license.
• Access to insured and reliable transportation.
• Ability to travel within the surrounding counties.

Responsibilities

• Monitor the personal care, companionship and skilled nursing services provided by direct care staff.
• Assist with coordinating clinical services and ensuring quality care services are provided to clients.
• Perform in-home assessments of clients to determine their support and clinical needs.
• Conduct scheduled supervisory visits to clients' homes.
