Plans, organizes and directs the Discharge Facilitators staff who are responsible for the operational activities of the Transitions of Care for Case Management department. Assures appropriate staffing and manages the financial performance of the area being managed. Responsible for completing job evaluations, hiring decisions, providing input for budgeting and meeting productivity goals. Responsible for developing post-acute relationships and fast contracting/invoicing. Responsible for FAST program invoicing/tracking transportation. Accountable for strategic planning, implementation, evaluations, and revision of systems that support the Case Management Department and organizational goals. Maintains 24-hour accountability for his/her area functions.
