About The Position

The Supervisor-TCL Care Management oversees non-licensed TCL Care Managers who assist members utilizing behavioral and physical health services within the Alliance Health Plan. This role ensures that care managers adhere to the principles of permanent supportive housing, aiming to stabilize tenancy and coordinate comprehensive services for members to live integrated lives in the community. The position is primarily remote, requiring the candidate to reside in North Carolina and travel as needed throughout the region.

Requirements

• Master's degree in Human Services or related field with three years of post-degree experience in care management or case management.
• Fully or provisionally licensed in North Carolina as LCSW, LCAS, LCMHC, LMFT, or LPA, or a Registered Nurse with a valid license in North Carolina.
• Experience working with individuals with developmental disabilities or traumatic brain injury.

Nice To Haves

• Experience with managed care and population health management.
• Certification in NACCM, NADD, CCM, or as a Community Health Worker.
