We are currently seeking a Case Management Supervisor RN to join our healthcare family. This role is pivotal in ensuring that our case management staff operates effectively and efficiently, providing the highest level of care to our patients. The Supervisor Case Manager will oversee the case management team, acting as a communicator, leader, assessor, planner, implementor, and evaluator. This position requires active participation in a collaborative program that reviews patient activities related to medical necessity for hospitalization, appropriateness of length of stay, and timely utilization of services. The Supervisor will also identify patient needs related to care following discharge, ensuring a smooth transition for patients back to their homes or other care settings. In this role, you will support the philosophy of HCA Houston Medical Center by facilitating cooperation among hospital staff in a multi-disciplinary approach to patient care. You will consistently provide outstanding service to all patients, physicians, fellow employees, and guests. Your responsibilities will include maintaining awareness of your own limitations and seeking guidance when necessary, prioritizing work activities based on service demands, and demonstrating an understanding of patient rights, including confidentiality and informed decision-making. You will also be responsible for performing technical skills proficiently, adhering to established standards, and assisting with the development of unit-specific policies and procedures to ensure smooth operations. As a Case Management Supervisor, you will participate in employee interviewing, hiring, and termination processes, provide input for performance evaluations, and collaborate with management to ensure adequate staffing. You will also be accountable for reviewing and monitoring hospital utilization of resources, appealing Medicare/Medicaid and Managed Care denials, and facilitating discharge planning utilizing social services and community resources. This role is essential in maintaining compliance with regulatory organizations and preparing case management reports for administration and committees. Your expertise will contribute to the development and implementation of a physician advisor program, ensuring that our hospital meets the highest standards of care and operational excellence.
