About The Position

The Supervisor, Post Acute Complex Care is a multi-market leader that oversees the Post Acute Care Coordinators. The success of this role is determined by their team’s ability to manage timely completion of daily census, and management of post acute patients until they return to the care of PCP to prevent hospital arrivals. Other key relationships include acute case managers, hospitalists, physician specialists, skilled nursing, and rehabilitation facility staff. The incumbent is responsible for training new Post Acute Coordinator (PHC) and quality assurance of their work of a defined region. Additional duties may include development of training materials, development of quality assurance processes, contribution to team growth and data quality strategy, Post Acute Coordinator interviewing and function coverage in regions outside of their usual Post Acute Complex Care teams. This incumbent will fill a Post Acute Coordinator role for a specific regional Post Acute Complex Care region as needed up to 50% of their work time.

Requirements

  • 2 + years-experience in Post Acute Care Coordinator or similar role outside of company is preferred.
  • Demonstrated leadership skills managing other and/or developing program standard operating procedures, quality assurance processes and/or training processes preferred.
  • Established track record of excellent data management skills.
  • High Level of proficiency with Microsoft Office Suite, including intermediate Word, Excel & PowerPoint skills.
  • Strong interpersonal, communication and critical thinking skills are required.
  • Ability to work autonomously is required.
  • Spoken and written fluency in English.

Nice To Haves

  • Previous referral experience is preferred.
  • Previous training experience is preferred.
  • Previous experience working in a medical setting with possible intake in a hospital or working with coordination of care with patients.
  • Coordination of care for discharge planning to include discharge appointments, home health and durable medical equipment (DME).

Responsibilities

  • Supervises a team of Post Acute Care Coordinators to deliver on maintain census integrity, supporting discharge coordination, and managing patients post hospitalization until they are reengaged into their PCP care.
  • Training of new Post Acute Coordinators (face to face or virtual).
  • Actively involved in the full employment cycle of direct reports, including but not limited to: participation in the recruitment process, annual performance appraisals, day-to-day oversight of staffing/scheduling needs, weekly time, time off approval, team vacation coverage plan, problem solving, exit interviews and termination/counseling decisions.
  • Develops training materials and standard operating procedures for Post Acute Coordinator role.
  • Provides quality assurance oversight for Post Acute Coordinator role in a defined set of regions.
  • Participate in Complex Care data quality and team growth strategy.
  • Accountable for the performance and execution of transitional care, acute, and post-acute partnerships.
  • Ensuring successful operation of Event Notification System (ENS) alerts including ensuring successful data capture from our Health Information Exchanges and payor census files.
  • Responsible for quarterly audit of authorization files to ensure accurate capture of IP/Obs/Denials in HITS.
  • Support maintenance of ENS/DNS integration to our system in collaboration with IT/Analytics team.
  • Assist with other projects as needed by HCT leadership.
  • Documents all aftercare and transition information in member record timeliness reviewing notification at the start of the day as well as through the day for accurate inpatient census.
  • Establish and maintain flow of communication within the transitional care team.
  • Secures discharge and transition plans from discharging facilities and evaluating plans to ensure compliance with clinical and quality requirements.
  • Serves as a bridge between inpatient and outpatient treatment providers.
  • Notifies health plan partner of all inpatient admissions and discharges and engaging health plan staff in discharge planning activities as needed in conjunction with the assigned care manager.
  • Documents inpatient/obs capture rate >95% Inpt/obs accuracy - >90%.
  • Educates and support centers as needed.
  • Performs other duties as assigned and modified at manager’s discretion.
