About The Position

The purpose of this position is to care manage patients/residents as assigned, collaborating with and supervising other disciplines and services. Such collaboration must be in accordance with current federal, state and local standards and guidelines to ensure that the highest degree of quality is maintained at all times.

Requirements

• Credential: RN: Registered Nurse
• Credential: ACLS: Advanced Cardiac Life Support. Equivalent Experience: Within 12 months of hire or transfer
• Credential: BLS: Basic Life Support. Equivalent Experience: Within 1 month of hire or transfer. Qualification Source: AHA
• Credential: FINGERPRINT: Fingerprinting
• Credential: TEAM ADVANCED: Techniques for Effective Aggression Mgt. Equivalent Experience: Within 6 months of hire or transfer (Employees who are in this position on or before 12/31/2023 will have 24 months to complete this certification)
• Education: Associates Degree in Nursing required.
• Read, write, speak and understand the English language.

Nice To Haves

• Bachelor's Degree in Nursing at time of hire or transfer to position preferred.
• Demonstrated leadership abilities; able to work under stressful situations; and displays use of professional verbal and written communication skills with integrity and initiative.
• Ability to make independent decisions when circumstances demand.
• Ability to deal tactfully with personnel, residents, family members, visitors, government agencies/personnel and general public.
• Willing to seek out new methods and principles and incorporate them into existing practice.

Responsibilities

• Responsible for the assessment, development and ongoing changes of patient/resident care plans, and quality outcomes.
• Participates in direct patient/resident care as needed.
• Ensures appropriate care plans are initiated and updated with condition change, and documentation supports care provided.
• Daily rounding with staff and patient/resident.
